Where can I create a form or survey?

You have three options when needing to create a form:

Basic



The first option is to use Google Forms.

Google Forms is a tool that allows collecting information from users via a personalized survey or quiz. The information is then collected and automatically connected to a spreadsheet. The spreadsheet is populated with the survey and quiz responses.

Advanced



If the Google solution isn't sufficient for your needs you can use Qualtrics.

Qualtrics is a web-based survey tool available for use by all University of Maryland faculty, staff, and students to support teaching and research on campus.

Qualtrics allows you to create, test, and execute high-quality on-line surveys. It provides a wide range of question types and formats, different strategies for distributing and following up with targeted respondents, and a suite of analysis and reporting tools.

To get started, you will need to log in here with your Directory ID and password. Accounts will typically be created within 48 hours.

Wordpress



If your team has a Wordpress website managed by the Department you have the option of using two plugins that give you simple but powerful form and workflow tools. Please contact meIT for installation.

Gravity Forms

Gravity Flow



Allows you to turn a Gravity Form into an efficient workflow.

Feedback and Review Loops


Implement review loops allowing users to edit their values before re-submitting for approval. Use workflow conditional logic to branch the process or skip steps.
